Anyone who receives a COVID vaccine in Oklahoma can  print/download their shot records from OSIIS (Oklahoma State Immunization Information System) at:

https://osiis.health.ok.gov/osiis_public/Application/PublicPortal

No login required.  Information must be entered into an online form and must be an exact match to what is in OSIIS.  Please use the cell phone number or email address that is attached to the OSIIS record.  If the email address isn’t listed on the record, the system will not return a record.
