Alumni Awards Nomination Form
Five alumni awards are available to be presented annually by the OU College of Pharmacy Alumni Affairs Board. The Alumni Association calls for award nominations each February and selection is made during the Spring Alumni Affairs Board meeting. The awards acknowledge those alumni who have made important contributions to the profession and who exemplify the values of the College. Nominations must be non-anonymous and include a minimum of one paragraph detailing why the nominee is deserving of the award. Please include the nominee's name (including degrees/credentials), graduation year, email, mailing address, and reason behind the nomination. It is important to specify how the nominee meets the stated award criteria. Submissions will be accepted until March 25 online via the form below or via email to cassidy-roberts@ou.edu.
